This research article examines the independent contribution of chronic traumatic encephalopathy (CTE) neuropathology to cognitive symptoms and dementia. The study analyzed a sample of 614 brain donors, with 366 diagnosed with autopsy-confirmed CTE and 248 without. The findings indicate that CTE stage IV is associated with significantly increased odds of dementia, while stage III also shows a notable association. Specifically, individuals with stage IV CTE have 4.48 times the odds of having dementia compared to those without CTE. Additionally, higher CTE stages correlate with greater informant-reported cognitive symptoms. However, no associations were found between CTE and mood or behavioral symptoms. The research highlights the neuropathological characteristics of CTE, including the presence of hyperphosphorylated tau protein, and discusses the challenges in accurately diagnosing CTE during life. It also emphasizes the need for further investigation into the thresholds of CTE neuropathology necessary for symptom manifestation.
