2025 Community Health Needs Assessment Report

This document is a Community Health Needs Assessment report conducted by AdventHealth Daytona Beach from March to December 2024. The assessment aimed to engage public health and community stakeholders, particularly focusing on low-income, minority, and underserved populations. It sought to understand the community's health issues, health behaviors, risk factors, and social determinants impacting health. The report details the collaborative process involving various local organizations and community members, which included surveys, interviews, and focus groups to gather input. The findings identified the top eight health needs within the community, which were then prioritized by a Hospital Health Needs Assessment Committee (HHNAC) based on available resources and community impact. The report concludes with the approval of the findings by the AdventHealth Daytona Beach board and outlines the next steps for developing a 2026-2028 Community Health Plan to address the prioritized needs. The top three needs identified are mental health, drug and alcohol use, and community support.
