Heart Failure Telemonitoring Program Outcomes

This peer-reviewed study evaluates the outcomes of the Heart Failure Telemonitoring Program implemented by Geisinger Health Plan (GHP). The study analyzes claims data from GHP Medicare Advantage members enrolled in the program from January 1, 2007, to October 31, 2012. It assesses the program's impact on all-cause hospital admission rates, readmission rates, and total cost of care. The findings indicate significant reductions in the probability of all-cause admissions and readmissions, with an odds ratio of 0.77 for all-cause admissions and 0.56 and 0.62 for 30-day and 90-day readmissions, respectively. Additionally, the study reports an 11.3% decrease in the cost of care and an estimated return on investment of 3.3. The telemonitoring program is presented as a potentially effective tool for managing elderly patients with heart failure, allowing for early detection of deteriorating conditions and reducing the need for urgent medical interventions.
