Reducing Hospitalization and Cost for Medicaid Beneficiaries with Heart Failure

This document is an observational study that assesses the impact of a remote monitoring and virtual care management program for Medicaid members diagnosed with heart failure. The study aims to evaluate whether this program affects all-cause hospital admissions, 30-day readmissions, total healthcare costs, and return on investment (ROI). The design includes a retrospective analysis of a non-randomized intervention cohort with two propensity matched observational control arms. The intervention consists of a remote home monitoring program that utilizes biometric measurements and automated coaching, combined with virtual nurse coaching. Participants include adult patients with stage II or higher heart failure enrolled in Managed Medicaid insurance plans. Key results indicate that the intervention group experienced fewer all-cause admissions and a significantly lower 30-day readmission rate compared to controls. Additionally, the program demonstrated an average savings per member per month, resulting in a favorable ROI. The findings support the efficacy of remote monitoring in managing heart failure and reducing associated costs.
