This research article investigates the experiences of medically complex patients, their family caregivers, and health care team members involved in Accountable Care Organizations (ACOs) compared to traditional fee-for-service models. Conducted by the American Heart Association, the study involved a literature review and interviews with 27 participants, including patients, caregivers, and health care professionals. The findings indicate that ACOs provide enhanced patient care and outcomes, attributed to strong primary care relationships, care coordination, and a team-based approach. Participants reported improved engagement, communication, and access to resources addressing health-related social needs. The study highlights the benefits of ACOs in fostering a better practice environment for health care providers, allowing them to focus on comprehensive patient care. Additionally, it emphasizes the importance of technology in tracking patient outcomes and facilitating communication among health care team members. The results advocate for the continued expansion and refinement of the ACO model to enhance health care delivery and patient outcomes.
