This case study details the transformation of Prowers Medical Center's quality improvement processes through the implementation of i2i Population Health's solutions. The hospital, facing significant challenges due to limited resources, geographic isolation, and regulatory pressures, needed to meet complex reporting requirements while managing a small quality department. The case study outlines how Prowers Medical Center adopted i2iTracks® and PRiZiM® to automate data abstraction and streamline quality workflows. As a result, the hospital was able to enhance data governance, foster a culture of accountability, and advance health equity by using demographic data to identify care gaps. The implementation led to significant improvements in quality measures, increased efficiency, and a shift from reactive to proactive care. The case study also emphasizes the importance of collaboration among all stakeholders in quality improvement efforts and highlights the hospital's plans to expand its use of i2i for ongoing population health initiatives.
