This case study details the Eastern Plains Healthcare Consortium's (EPHC) efforts to enhance rural health in Colorado through strategic collaboration and data integration. Formed in 2015, EPHC comprises 15 independent critical access hospitals and two large health systems, evolving from a networking group to a sophisticated hub for shared services and performance improvement. The study outlines the challenges faced by EPHC, particularly with the Colorado Hospital Transformation Program, which required coordinated efforts to meet Medicaid reporting requirements. The consortium implemented i2i Population Health solutions to aggregate data from disparate electronic health record systems, enabling performance benchmarking and quality improvement. Key outcomes included improved care coordination, enhanced clinical documentation, and better decision-making processes. The case study emphasizes the importance of collaboration, standardized data, and clinical leadership engagement in achieving significant improvements in rural healthcare delivery.
