Impact of Transitions of Care on Community Health

This case study examines the impact of transitions of care on community health, specifically focusing on the collaboration between the Indiana Primary Healthcare Association (IPHCA) and Azara Healthcare. The initiative, known as Project UTILIZE, aims to enhance population health management by integrating health information exchange (HIE) data into a comprehensive analytics platform. The document outlines the challenges faced by community health centers (CHCs) in Indiana, including limited access to healthcare data and increased costs due to emergency department utilization. It presents the solutions implemented, such as the Azara DRVS and Transitions of Care Module, which facilitate improved data collection and analysis.
