Ohio’s Data-Driven Health Transformation Case Study

This case study details the Ohio Association of Community Health Centers' (OACHC) initiative to improve population health through a data-driven approach. OACHC, representing 59 Federally Qualified Health Centers in Ohio, aimed to enhance healthcare quality for underserved populations. The initiative focused on establishing a data-sharing platform to facilitate the exchange of clinical, operational, and financial data among health centers. This platform, known as the Ohio Data Integration Platform (ODIP), was developed in partnership with Azara Healthcare. The case study outlines how ODIP enabled real-time performance tracking, collaborative interventions, and enhanced data sharing, ultimately leading to significant improvements in health metrics. Notably, from 2020 to 2022, the initiative resulted in enhanced preventive care and chronic disease management, with Ohio's Community Health Centers achieving top rankings in several health measures. The study emphasizes the importance of data in driving healthcare improvements and the commitment of OACHC to addressing health equity and enhancing care delivery.
