This case study details the optimization of the urine culture process at Texas Children's Hospital, focusing on the implementation of time-based urine culture analysis supported by MALDI-TOF technology. The objective was to improve turnaround times (TAT) for urine culture results, which are critical for patient care and antibiotic stewardship. The study outlines the methods used, including a systematic gap analysis to identify inefficiencies in laboratory workflows and the introduction of synchronized readings across shifts. The results demonstrate a significant decrease in TAT by 15 hours for positive cultures and a sustained reduction of over 20 hours for negative cultures over seven years. The findings indicate that the new approach not only enhances lab efficiency but also allows for timely clinical decision-making, ultimately benefiting patient management. The project was recognized with the GJ Buffone Pathology Quality Improvement Award, highlighting its success in achieving its objectives through targeted improvements in laboratory processes.
