This research article investigates the feasibility and effectiveness of telecare consultations in a nurse-led post-acute stroke clinic. The study employs a quasi-experimental design, assessing stroke survivors who received telecare consultations over three months. The objectives include evaluating the integration of telecare into routine care for stroke survivors, aiming to enhance their health monitoring and self-care abilities post-discharge. The results indicate that 92 stroke survivors participated, with a notable drop-out rate of 27%. Participants reported the program as time-efficient and cost-effective, alleviating health-related concerns. Improvements were observed in daily living activities and quality of life at the three-month follow-up. The findings suggest that telecare can serve as a viable alternative to traditional consultations, potentially increasing appointment attendance and patient satisfaction. The study advocates for policy adjustments to support telecare, promoting accessible healthcare for stroke survivors and possibly extending to other chronic conditions.
