This case study details the implementation of the ADVANTICS HeartLogic™ solution at the Liverpool Heart and Chest Hospital (LHCH) to optimize remote patient monitoring for heart failure (HF) alert management. LHCH, serving a catchment area of approximately 2.8 million people, faced challenges related to rising demand and limited infrastructure for monitoring HF patients. The study outlines a two-phase advisory program that included baseline analytics, process mapping, and stakeholder mapping to identify areas for improvement. The implementation of the HeartLogic™ digital solution aimed to enhance the efficiency of HF alert management, with significant outcomes observed over a six-month period. Key improvements included a 77% reduction in the average time required to manage an HF alert and a 21% reduction in the time patients spent in HF alert. The study emphasizes the collaboration between LHCH and Boston Scientific to advance remote patient monitoring services and highlights the potential for broader adoption of the HeartLogic™ solution across the Integrated Care System.
