This research article presents a comparative analysis of three chimeric antigen receptor (CAR) T-cell therapy strategies for treating autoimmune diseases, specifically systemic lupus erythematosus (SLE). The article outlines the methodologies of autologous, allogeneic, and in vivo CAR T-cell approaches, detailing their clinical designs, safety profiles, and efficacy based on early-phase clinical studies. Each strategy is evaluated for its manufacturing processes, with emphasis on the engineering of T cells and the associated risks of graft-versus-host disease (GvHD) and host-mediated allorejection. The article documents the results of clinical trials involving small patient cohorts and discusses the translational challenges faced in the development of in vivo CAR engineering. The findings suggest that while all three strategies show promise, further research is necessary to optimize their therapeutic potential and address safety concerns.
