This case study outlines a telemonitoring project initiated by the cardiology department of ASZ Aalst, an innovative hospital in Belgium, aimed at improving the management of patients with chronic heart failure. The project commenced with a pilot phase that provided continuous follow-up for patients from home over six months. The primary objectives included enhancing patient awareness and involvement in their therapy, decreasing complications and hospitalizations, and empowering patients to take control of their health. The program utilized a HomeHealth App with Bluetooth-connected devices for real-time monitoring, while ASZ Aalst employed the e-Care telemedicine platform for 24/7 patient oversight. The project was tailored through a collaborative approach with Comarch, ensuring that the solution met the specific needs identified by the hospital. Comprehensive training was provided to project leaders and nurses to facilitate effective use of the platform and patient education.
