This case study outlines the process undertaken by a major health system in the Greater New York metropolitan area to transition thousands of patient records into a centralized Electronic Health Record (EHR) system. The health system faced challenges due to the diverse formats of patient records from acquired medical practices, which included both paper and electronic formats. To address these challenges, Docufree® provided consultancy services and developed a strategic plan for the migration of existing and acquired practice information to one EHR. The solution involved a staged approach that included assessing records, determining their digital or hard copy destinations, and converting paper charts to digital formats on demand. The initiative aimed to create a unified, compliant digital workplace and improve patient outcomes through better access to medical records. The case study also details the successful transition of over 300,000 patient medical records, the repurposing of filing rooms into patient exam rooms, and the financial benefits realized by the healthcare system.
