This case study details the implementation of a colorectal cancer (CRC) screening program at Sanitas Medical Center, aimed at increasing screening rates among eligible patients. The program was initiated in response to low screening rates, which were below the national goal of 80%. Sanitas collaborated with Exact Sciences to develop a multidisciplinary approach that included patient education, tailored navigation, and outreach to both English- and Spanish-speaking populations. Key interventions included the use of electronic health record (EHR) systems to identify unscreened patients, the establishment of standing orders for Cologuard tests, and the implementation of a sustainable gap-closure program. These efforts resulted in a 111% increase in CRC screening rates over a four-year period, demonstrating the effectiveness of targeted outreach and the integration of technology in healthcare delivery. The program also addressed barriers such as language differences and patient misconceptions about screening, ultimately improving access to care for a diverse patient population.
