This case study details the efforts of the Bedford-Stuyvesant Family Health Center (BSFHC) to enhance colorectal cancer (CRC) screening rates among underserved populations. It outlines the challenges faced in increasing screening rates, including patient confusion regarding test instructions and logistical barriers. The study highlights a collaboration with Exact Sciences to implement the Cologuard test as a primary screening method, which has been integrated into the health center's electronic health record system. The initiative involved a comprehensive team approach, emphasizing patient education and support through various communication channels. As a result of these efforts, CRC screening rates improved from 48% in 2022 to 59% in 2024, with a notable increase in adherence among younger patients. The case study also discusses ongoing strategies for continuous improvement, including the refinement of outreach methods and the expansion of in-person test completion opportunities to further enhance patient engagement and screening rates.
