This case study details the implementation of a data aggregation and entry tool for an Accountable Care Organization (ACO) and a Clinically Integrated Network (CIN) in New Jersey. The organization comprises independent and employed physicians serving a population of 45,000. The primary objective was to facilitate the Medicare Shared Savings Program (MSSP) GPRO Quality reporting to enhance patient care through quality and cost improvements. The situation arose when the ACO needed to transition from their previous vendor to The Garage just before the reporting season in December 2021. The Garage successfully developed and launched a Care Gaps application within six weeks, which included a comprehensive Quality Payment Program (QPP) reporting workflow. The outcome of this implementation was that the ACO's team, consisting of only two end users, completed their reporting ahead of schedule and achieved scores that were 3-4% higher than the previous year, demonstrating the effectiveness of the new platform.
