This document is a case study detailing the implementation of a care gap management solution within an Accountable Care Organization (ACO) serving approximately 9,000 lives in Eastern Idaho. The ACO has adopted a unique model that integrates care managers into each of its 16 organizations to enhance population health management. A key initiative for 2022 and 2023 was to address care gaps proactively, which necessitated an effective technological solution. The Bridge's Care Gaps application was introduced to unify electronic medical records and payer data, providing real-time care gap lists and quality scores. The document outlines the training and workflows established to facilitate the adoption of this application. Notably, the average daily care gap closure rate improved from 6.3 to 9.6 measures per day following the implementation. The case study emphasizes the positive user experience and the enhanced visibility into performance metrics, which fostered a data-driven approach to quality improvement within the ACO.
