The hospital, which serves over one million patients annually, established an in-house retail pharmacy to provide convenient prescription fulfillment services. The primary objectives included decreasing hospital readmission rates and ensuring patients were discharged with their medications to improve compliance. The Meds to Go program allows patients to request medications directly from their bedside through SMART TV prompts, facilitating a streamlined process for enrollment and communication with pharmacy staff. Additionally, a pharmacist visits each patient to provide personalized counseling and address any questions prior to discharge. The implementation of this program resulted in increased patient satisfaction, improved medication safety, and a notable growth in net pharmacy profit over three years.
