Digital Solutions for HIV Care Improvement

This report summarizes the outcomes of the CWJWP project aimed at enhancing the care of people living with HIV (PLWHIV) who are in early frailty or have co-morbidities. The project, conducted by Chelsea and Westminster Hospital NHS Foundation Trust in collaboration with Gilead Sciences, sought to explore how digital technologies could improve patient care and outcomes. The project commenced in December 2020 and concluded in May 2023, encompassing various activities such as capturing patient feedback, mapping current care pathways, and conducting workshops to identify potential digital solutions. The findings from co-designed questionnaires informed the development of a minimum viable product, specifically a clinician dashboard designed to collate patient information and identify those at risk of frailty. The report also discusses lessons learned regarding the integration of digital solutions within existing NHS systems, emphasizing the importance of compatibility with electronic patient records for successful implementation.
