This document is an executive summary of a joint working project between GlaxoSmithKline (GSK) and the East Kent GP Confederation focused on the management of Chronic Obstructive Pulmonary Disease (COPD) patients identified as high risk. The project aims to optimize the management of these patients by assisting primary care in identifying and prioritizing individuals with risk factors, aligning with local guidelines and prescribing strategies, and promoting sustainable practices. The project is set to run from April 2023 to July 2024, with GSK funding patient reviews across up to 35 practices at a total estimated cost of approximately £380,679. The NHS will contribute resources valued at around £200,638 without any monetary transfer. The intended benefits include improved patient outcomes, better management of COPD, and enhanced healthcare experiences, while also supporting the NHS's value-based healthcare objectives and promoting sustainability in prescribing practices.
