This document is a summary report of the 'Legacy across Black Country' Joint Working Project conducted by GlaxoSmithKline (UK Ltd) and Black Country Integrated Care Board (ICB) from January 2023 to November 2023. The project's objective was to standardize patient care for Chronic Obstructive Pulmonary Disease (COPD) in alignment with national and local guidelines, while also reducing the practice burden associated with long-term condition management. The project involved validating the COPD disease register, ensuring compliance with management guidelines, and supporting NHS priorities. A total of 31 GP practices participated, with 679 patients reviewed, and the results indicated a mix of face-to-face and remote consultations. The report also outlines pharmacological and non-pharmacological interventions undertaken during the project, as well as lessons learned regarding practice recruitment and data analysis. Overall, the project aimed to enhance the quality of primary care COPD management.
