EHR Migration Strategy for Healthcare Organization

This technical report outlines the process undertaken by a multi-state healthcare organization to migrate approximately 2 million patient charts from over a dozen legacy electronic health record (EHR) systems into a unified platform. Initially scoped for 1.5 million charts, the project expanded due to the addition of more systems. The migration faced challenges such as the COVID-19 pandemic, facility delays, and a cyberattack in 2024. The report details the methodology established for the project, which included data analysis to assess sources and readiness, planning to align critical data elements with the target EHR, and training abstractors to meet quality standards. The team operated continuously during peak periods to manage the scale of the project. The results indicate a successful abstraction model that achieved approximately 98.6% quality assurance scores, with operational continuity maintained and minimal clinical disruption. This model is now being used as a template for other U.S. health systems.
