This document is a case study detailing the implementation and outcomes of the Clayton County Community Treatment Program (CTU) and its successor, the 4C Program. The initiative was developed in response to high 911 call volumes and limited healthcare resources in Clayton County, Georgia. The CTU was launched in 2016 to provide mobile integrated healthcare, but it was later revamped into the 4C Program in 2022, focusing on reducing repeat 911 calls and addressing health disparities. The program utilized ImageTrend's Community Health module and Continuum solutions to enhance patient management, streamline data, and improve field operations. As a result, patients enrolled in the 4C program significantly reduced their 911 usage, and the department reported its first annual call volume decrease despite population growth. The program also fostered partnerships with community organizations to address social determinants of health, contributing to improved patient outcomes and community health.
