Mobile Integrated Healthcare Program Implementation

This case study outlines the implementation of the Mobile Integrated Healthcare (MIH) program by the West Allis Fire Department, aimed at reducing high-utilizer calls and addressing community health needs. The department, serving a population of approximately 60,000, faced challenges with an average of 25–28 high-utilizer calls per month, which strained resources. The MIH program specifically targeted individuals calling 911 three or more times monthly, focusing on social determinants of health, substance use disorder, and fall prevention. Utilizing ImageTrend’s Elite ePCR and Community Health solutions, the department developed customizable data insights and streamlined reporting processes. The program has successfully reduced high-utilizer calls by approximately 80%, from an average of 25–28 to about 5.3 per month. The case study also discusses ongoing efforts to refine data management processes and enhance community health outcomes through proactive care and targeted follow-up initiatives.
