This case study details the Mobile Integrated Healthcare (MIH) program implemented by the West Allis Fire Department to address high-utilizer calls and community health needs. The department, serving a population of around 60,000, faced challenges with approximately 12,000 annual calls, particularly from individuals calling 911 multiple times a month. The MIH program aims to reduce these calls by focusing on social determinants of health and managing specific needs related to substance use disorder (SUD) and fall prevention. Utilizing ImageTrend's Elite ePCR and Community Health solutions, the department has developed customizable reports and remote monitoring capabilities to track high-risk patients. The results have shown a significant reduction in high-utilizer calls, dropping from an average of 25-28 per month to about 5.3, representing an 80% decrease. The program also emphasizes efficient reporting and improved patient outcomes through targeted initiatives and data management enhancements.
