This case study outlines the engagement between a large multi-site hospital system and Impact Advisors to enhance Hierarchical Condition Category (HCC) documentation and improve performance in the Medicare Shared Savings Program (MSSP). The health system faced challenges with key performance indicators, prompting the need for a structured and data-driven approach. Impact Advisors conducted a comprehensive assessment, which included stakeholder interviews and baseline measurements, to identify areas for improvement. The program incorporated workflow redesign, Epic EHR optimization, and provider education to strengthen governance and enhance documentation practices. The engagement led to significant improvements, including increased visibility into performance and enhanced provider engagement. The hospital system achieved substantial financial benefits, realizing $40.0 million in risk adjustment value and $16.1 million in new shared savings, alongside the documentation of 2,768 new HCCs. Overall, the case study demonstrates the effectiveness of a coordinated approach in driving sustainable improvements in HCC documentation.
