This document is a research article that compares the effectiveness of transforaminal epidural steroid injection (TFESI) and high-volume lumbar erector spinae block (ESPB) in treating patients with low backache and radicular pain. The study involved a randomized clinical trial with 60 participants aged 18 to 50 years, who were assigned to either TFESI or ESPB groups. The primary outcome measured was pain relief assessed by the numeric rating scale (NRS) at three months post-procedure. Secondary outcomes included NRS scores at one hour and one month, the Modified Oswestry Disability Index (MODI), and the incidence of complications. Results indicated that TFESI provided significantly better pain relief compared to ESPB at all measured time points. However, while TFESI showed superior efficacy, it also carried a higher risk of complications, suggesting that ESPB may be a safer alternative in certain cases. The study highlights the need for careful consideration of treatment options for chronic low back pain.
