This research article presents the development of an evaluation tool for the urban family medicine program within the Iranian health system. The study employs a mixed-methods approach, beginning with a literature review followed by qualitative interviews with experts and stakeholders. The objective is to create a comprehensive evaluation framework that identifies key indicators for assessing the program's effectiveness. A total of 103 indicators were derived from the literature, categorized into structural, process, and output indicators. Additionally, 17 indicators emerged from expert interviews. The evaluation tool was refined through two rounds of the Delphi method, resulting in 70 final indicators across eight dimensions, including service delivery and community participation. The findings emphasize the importance of a systematic evaluation to enhance service quality and patient satisfaction in urban family medicine. The study concludes that a well-structured evaluation tool is essential for identifying strengths and challenges within the program, ultimately contributing to improved health outcomes.
