This document is a case study detailing the efforts of Liaison Care in addressing the backlog of Continuing Healthcare (CHC) reviews for an Integrated Care Board (ICB). The ICB faced challenges with overdue reviews that could lead to inappropriate care packages for patients. Liaison Care was contracted to conduct clinical reviews of the backlog, ensuring that care provision was aligned with individual needs and that funding was appropriately allocated. Over a period of seven months, 360 individuals were reviewed across three localities and two Local Authorities, resulting in a significant reduction of £2.9 million in unnecessary spending. The case study outlines the completion of 336 person-centred reviews and the formation of 86 multidisciplinary teams (MDTs), with 41 MDTs identifying reduced care needs.
