This technical report presents a comparative longitudinal cohort analysis of risk-adjusted readmission rates for skilled nursing facilities (SNFs) associated with Penn Medicine’s Lancaster General Hospital (LGH) from September 2017 to December 2022. The analysis focuses on SNFs that implemented an interventional analytics (IA) platform compared to those that did not, as well as to other SNFs in Pennsylvania and nationwide. The study utilizes data from the Centers for Medicare & Medicaid Services (CMS) to assess readmission rates and quality metrics. The findings indicate that LGH facilities utilizing the IA platform had significantly lower readmission rates compared to their peers, with a notable divergence in performance metrics. Specifically, in the latest reporting period, SNFs with IA exhibited readmission rates that were substantially lower than those without IA and other cohorts. The report concludes that the implementation of IA in SNFs is associated with improved outcomes and suggests further investigation into the efficacy of such technologies in reducing hospital readmissions.
