This case study details the collaboration between Saint Peter’s University Hospital and Lightbeam Health Solutions to address social determinants of health (SDOH) among patients. The primary objective was to identify high-needs patients and their specific SDOH risk factors using an AI model. The implementation aimed to enhance staff productivity through efficient referrals to existing social service programs and to measure the clinical and financial impacts of their health equity initiatives. The AI model predicted patients at risk of high utilization in care based on social vulnerability data, enabling targeted interventions. The study reports a 7.1% reduction in emergency department visits among the intervention group compared to a matched control group. Key strategies included integrating SDOH screenings into routine care, hiring bilingual health educators, and establishing a food market to support patients. The results indicate promising outcomes for improving patient access and health outcomes while ensuring financial sustainability.
