This case study details the experience of Mankato Clinic, a physician-owned multi-specialty practice, as it transitioned to a value-based care model while managing both high-risk and rising-risk patient populations. The clinic faced challenges in improving care quality and reducing costs, particularly as it expanded its patient base under downside risk contracts. To address these challenges, Mankato Clinic partnered with Lightbeam Health Solutions to implement a Deviceless Remote Patient Monitoring (RPM) system called CareSignal. This system enabled the clinic to engage patients with chronic and mental health conditions through automated communications, allowing for timely interventions. The study outlines the goals of increasing chronic care management (CCM) enrollment, reducing avoidable utilization, and scaling care management. Results indicated significant improvements, including a reduction in emergency department visits and cost savings of $1.5 million. The implementation of Deviceless RPM facilitated enhanced patient engagement and proactive care management, demonstrating its effectiveness in supporting the transition to value-based care.
