This case study outlines the development and impact of the GRADE (Grading of Recommendations Assessment, Development and Evaluation) approach, which assesses the quality of evidence and the strength of recommendations in systematic reviews. The research began in 2000, with implementation starting around 2004. The GRADE working group, consisting of international collaborators, aimed to create a transparent and systematic method for evaluating the confidence in findings from systematic reviews. The GRADE tool has become a gold standard in organizations such as the Cochrane Collaboration and the WHO. The study details the iterative process used to develop GRADE, including user testing and feedback, which led to the creation of Summary of Findings tables that enhance reporting clarity. The GRADE approach has been widely adopted, influencing numerous guidelines and systematic reviews globally, and has facilitated further methodological developments, including GRADE CERQual for qualitative evidence synthesis. The impact of GRADE extends to both producers and users of systematic reviews, improving transparency and decision-making in health care.
