This case study details a pilot program implemented by UCI Health aimed at improving communication between nurses and providers in the Tower Medical Surgical units. The program was initiated to address identified gaps in communication that negatively impacted patient experience scores. Over four months, the pilot focused on enhancing timely communication of care plans among care teams, which was crucial for updating patients and their families. Feedback indicated increased engagement and collaboration among nursing and physician teams during the pilot. However, a subsequent four-month pause in the program led to a notable decrease in communication scores, highlighting the importance of sustained efforts in maintaining improvements. The study emphasizes the necessity of executive sponsorship and stakeholder involvement in such initiatives. The results of the pilot demonstrate the direct correlation between effective communication and patient experience metrics, including the Net Promoter Score.
