This case study details the implementation of Origami Risk at the University of Southern California (USC) to enhance risk management and compliance reporting across its clinical programs. The University Clinical Services (UCS) team, responsible for various healthcare services, identified the need to improve their risk management processes and promote a culture of safety. They expanded the use of Origami Risk, which was already utilized by USC's Office of Risk Management, to streamline incident reporting and peer review. The solution provided customizable forms and workflows, accessible via online and mobile platforms, facilitating widespread adoption among over 550 practitioners across 50 service sites. The case study outlines the challenges faced, including the need for a secure and efficient peer review process, and highlights the outcomes achieved, such as improved transparency, accountability, and timely reporting of incidents. The implementation has fostered a strong safety culture and enabled continuous improvement across clinical service areas.
