Enhancing Post-Acute Care Collaboration with Real-Time Data

This case study details the implementation of PAC Management by Physicians of Southwest Washington (PSW) to improve collaboration with skilled nursing facilities (SNFs) through real-time clinical insights. The document outlines the challenges faced by PSW, including delays in accessing crucial medical records and timely updates from SNFs, which hindered effective patient care and increased hospital readmission rates. The PAC Management solution provided by PointClickCare enables health systems and care teams to monitor patients throughout their post-acute care journey, facilitating timely interventions to prevent unnecessary readmissions.
