This case study details the initiative undertaken by Middlesex County as part of the State of New Jersey Destination 2040 Healthy, Safe, and Inclusive Communities initiative, aimed at supporting its aging population, particularly those aged 65 and older. The county recognized the challenges posed by social isolation and loneliness, which can exacerbate health conditions among older adults. To address these issues, Middlesex County engaged Pyx Health to provide innovative care management tools that combine technology with human support. The program utilized the Loneliness Index® to identify residents at risk of loneliness and facilitated outreach to connect them with essential resources. Through the pilot program, nearly 800 residents were contacted, resulting in 94 enrollments. The engagement provided valuable insights into the needs of the elderly population, leading to expanded outreach efforts. The initiative's success was acknowledged with a 2023 achievement award from the National Association of Counties, highlighting its impact on community health and well-being.
