This technical report presents an analysis of the evolving threat landscape and the inadequacies of traditional remediation approaches in cybersecurity. It discusses the findings from over one billion CISA KEV remediation records across more than 10,000 organizations from 2022 to 2025. The report highlights that the average Time-to-Exploit has decreased significantly, with adversaries exploiting vulnerabilities before patches are released. It emphasizes the structural limitations of manual remediation processes, which have failed to keep pace with the increasing volume of vulnerabilities. The report introduces concepts such as Average Window of Exposure and Risk Mass to illustrate the growing gap between the speed of exploitation and the speed of remediation. It advocates for a shift towards autonomous defense mechanisms, suggesting the establishment of a Risk Operations Center (ROC) that integrates intelligence and automated remediation processes. The report concludes that without this transition, organizations will continue to struggle with unresolved vulnerabilities and increased risk exposure.
