This research article evaluates the implementation of the National Primary Health Care Development Agency (NPHCDA) gateway for the Basic Health Care Provision Fund (BHCPF) across six northern states in Nigeria. Utilizing a mixed-method approach, the study combines qualitative and quantitative methods, including longitudinal surveys and key informant interviews, to assess the implementation status. The findings indicate that while the NPHCDA has accredited one Primary Health Care (PHC) facility per political ward in these states, challenges such as delays in business plan submissions and capacity gaps hinder optimal implementation. The research outlines factors contributing to success, including strong partner support and established financial management systems, while also highlighting areas needing improvement, such as government commitment and effective monitoring. The insights gained from this evaluation are intended to inform healthcare financing strategies in low- and middle-income countries, emphasizing the importance of addressing implementation challenges in PHC financing.
