This research article presents a study on the simulation of CD8+ T cell-mediated liver injury during ipilimumab administration using a quantitative systems toxicology model known as BIOLOGXsym. The study investigates the hypothesis that ipilimumab, an immune checkpoint inhibitor, may lead to liver injury by inhibiting CTLA-4, which is crucial for regulating T cell responses. The results indicate that 7% of patients treated with ipilimumab exhibit liver injury, characterized by elevated alanine aminotransferase (ALT) levels. The simulation involved over 500 virtual patients to capture variability in CD8+ T cell dynamics, including differentiation and killing rates. The findings suggest that lower T cell avidity is associated with delayed peak ALT levels, while higher initial naïve CD8+ T cell numbers correlate with increased peak ALT. This study provides initial evidence of how quantitative systems pharmacology can be utilized to explore mechanistic hypotheses related to immune-related adverse events in cancer therapies.
