This study presents findings from a cohort analysis involving over 24,000 members with chronic conditions across 34 Teladoc Health clients. The objective of the study was to measure the impact of an integrated chronic care management program on health outcomes for individuals managing multiple chronic conditions, including diabetes, hypertension, and mental health issues. The results indicate that enrolling in multiple integrated programs leads to significant improvements in health metrics. Specifically, members participating in additional programs experienced an average A1c reduction, a decrease in systolic blood pressure, and weight loss. The study highlights that mental health program enrollment is a key factor contributing to these improved outcomes. The findings suggest that comprehensive management of co-occurring conditions can enhance overall health and reduce healthcare costs, emphasizing the importance of a unified approach to chronic care management.
