This case study details the implementation of the TigerConnect Clinical Collaboration Platform at the University of Maryland Medical System (UMMS) to enhance patient transfer processes. With an expected increase in transfer requests, UMMS sought a modern communication solution to replace outdated methods. The case study outlines the transfer of a patient experiencing central retinal artery occlusion (CRAO) from Chestertown to Baltimore, emphasizing the time-sensitive nature of the condition. It describes how the integration of TigerConnect with the health system’s electronic health record (EHR) facilitated real-time communication among care teams, allowing for efficient coordination of care and timely treatment. The workflow enabled the patient to receive critical thrombolytic therapy within the necessary time frame. Feedback from ED Directors indicated a preference for the new workflow over traditional methods. The study concludes with plans for ongoing optimization of the transfer process to improve patient safety and provider satisfaction.
