This document is a case study that describes the Nurse Navigator program implemented by Magnolia Regional Health Center aimed at enhancing medication adherence among patients with congestive heart failure (CHF). The program addresses the challenges faced by patients in managing complex medication regimens, which often leads to inadequate adherence and adverse clinical outcomes. The case study outlines the process of identifying eligible patients during hospitalization, followed by follow-up calls at 72 hours post-discharge and again at 30 to 60 days post-discharge. The involvement of Nurse Navigators in counseling patients and families is crucial for ensuring adherence to guideline-directed medical therapy (GDMT). Early findings over a seven-month period indicate improved prescription fill rates and adherence to GDMT guidelines, particularly regarding new medications such as sodium-glucose cotransporter-2 (SGLT2) inhibitors. The case study concludes with a commitment to ongoing evaluation and potential submission of results to a peer-reviewed journal.
