This report is the 2021 progress evaluation of the Extended Care Pilot Program (EXCP), which aims to support young people transitioning from out-of-home care. The document outlines key findings from the third evaluation cycle, emphasizing the program's role in helping participants develop life skills and achieve positive outcomes. Notably, the evaluation indicates that EXCP participants are less likely to experience homelessness after leaving care and are more likely to pursue further education. Additionally, participants report feeling supported in building community connections and accessing health and wellbeing services. The report also highlights improvements in participant confidence across various domains over the evaluation cycles from 2019 to 2021. Future evaluations will utilize linked data to further assess the impact of the program on participants' use of health and social services compared to their peers in out-of-home care. The findings underscore the significance of the EXCP in fostering positive transitions for young people in care.
