Clinical Practice Setting Checklist

This document is a checklist designed for use during the Orientation Training of clinical practice placements. It outlines the responsibilities of the university supervisor, cooperating teacher, and candidate in ensuring that the placement meets all state and program requirements. The checklist includes various criteria that must be verified, such as the creation of a supportive teaching and learning climate, opportunities for collaboration and co-planning, and adherence to the University of Phoenix’s Six Phases of Gradual Releasing/Assuming of Responsibility model.
