This research article presents findings from a multidisciplinary and multinational eDelphi study aimed at identifying interventions to promote cost-effectiveness in adult intensive care units (ICUs). The study involved 61 experts from 21 countries who participated in a three-round modified Delphi process. The first round focused on qualitative feedback regarding cost-effectiveness criteria across four key domains of high-value healthcare. Subsequent rounds included both qualitative and quantitative assessments using a five-point Likert scale to reach consensus on proposed interventions. A total of 50 best practice considerations were identified, with 9 deemed 'critical' by the steering committee. These considerations emphasize the importance of a multidisciplinary model of care, staff training, ongoing quality audits, and strategies for end-of-life care, among others. The study highlights the need for effective resource utilization in ICUs while maintaining high-quality patient care, especially in light of challenges posed by the COVID-19 pandemic.
