This document is a guide that discusses how Health Information Exchanges (HIEs) can collaborate with public health organizations to utilize social determinants of health (SDOH) to enhance community outcomes. It outlines the challenges faced by public health organizations, particularly the issue of siloed databases that hinder access to essential information. The guide presents a case study of an independent nonprofit HIE in the Western U.S. that successfully employed SDOH data to improve analytics and reduce health inequities. It details the solution implemented, which involved using Verato UMPI™ for identity resolution, leading to the identification of eligible Medicaid patients and increased funding for programs like the Special Supplemental Nutrition Program for Women, Infants, and Children (WIC). The results included improved analytics, enhanced outreach, and a proactive approach to addressing health inequities, ultimately positioning public health organizations to better serve vulnerable populations.
