This case study outlines the implementation of Verato's hMDM platform by the Health Care Services Agency (HCSA) in California to support the Whole Person Care (WPC) initiative. The WPC initiative, launched in 2020, aimed to address the unmet needs of high-risk populations, including those experiencing homelessness, by providing comprehensive services. The HCSA recognized the need for a centralized Community Health Record (CHR) to integrate various data sources and create a 360-degree view of individuals. By utilizing Verato's referential matching technology, the HCSA was able to improve data accuracy and streamline care coordination across multiple organizations. The CHR enabled HCSA to quickly access essential information about individuals, including medical, social, and financial data, thereby reducing care gaps. The implementation of the hMDM platform has strengthened data partnerships and improved operational efficiency, allowing for better identification of individuals and enhanced service delivery to high-risk populations.
